  • Immunization Form

    Immunization FormThe Immunization Form must be completed before registering for classes at the university, although it is not necessary or considered during the admission process. Please be sure to complete and return this form to UCF's Student Health Services. You will not be allowed to register without having an Immunization Form on file.

    Instructions: Take the Immunization Form with you when you are immunized. Have the health care provider sign, date and stamp the Immunization Form in the documentation section. We recommend that you keep a copy of the completed form for your own records. Refer to the detailed instructions on the Immunization Form for more information about the required immunizations, exemptions from proving immunity, and acceptable forms of documentation.
